29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets: a) Front Yard On or about 08/04/2015, two masons were exposed to a struck-by hazard from falling brick, masons tools and buckets and were not provided with head protection. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet).

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level. Paragraphs (g)(1)(i) through (vii) of this section establish the types of fall protection to be provided to the employees on each type of scaffold: a) Front of House On or about 08/04/2015, two masons installing face brick on the front of a house where exposed to an 18 foot fall from the platforms of a supported scaffold which had not been provided with standard guardrails or other acceptable means of fall protection. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet).

29 CFR 1926.451(h)(2)(ii): A toeboard was not erected along the edge of platforms more than 10 feet (3.1 m) above lower levels for a distance sufficient to protect employees below: a) Front of House On or about 08/04/2015, two masons standing on the ground below the 18 foot high scaffold platform were exposed to a struck-by hazard from falling brick, tools and buckets for the scaffold platform was not provided with toe boards or some similar adequate means of restrain. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet).

29 CFR 1926.1053(b)(1): When portable ladders are used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access; or, when such an extension is not possible because of the ladder's length, then the ladder shall be secured at its top to a rigid support that will not deflect, and a grasping device, such as a grabrail, was not provided to assist employees in mounting and dismounting the ladder: a) Front of House - On or about 08/04/2015, workers were exposed to an 18 foot fall from the top of an aluminum extension ladder used to access a supported scaffold platform whose top rung was located 2 feet below the platform. No abatement certification or documentation required for this item.
