29 CFR 1926.20(b)(2):  The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s)    (a) Jobsite, On or about August 10, 2015 - the employer did not designate a competent person to inspect this site for jobsite hazards.  Observed hazards include; fall hazards from inadequate ladders, struck by hazards from improper and missing personal protective helmets and a trench that did not have proper protective systems installed.

29 CFR 1926.652(a)(1):  Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(c):  (a) Jobsite, On or about August 10, 2015 - the excavated trench that the employees were performing hand-digging in was not protected from a cave-in hazard.  The trench was approximately 11 feet wide by 15 feet long by 15 feet deep.

One employee was not wearing an approved helmet.  The other employee was not wearing any head protection.
