29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous: (a)Extreme Construction LLC at 4460 Tennyson Street, Denver, CO 80212: On and before August 8th, 2015, the employer did not ensure that the path to ground for three yellow extension cords was permanent and continuous when used with portable tools such as a Skilsaw, model HD77M, SN# HF-80935. The Skilsaw was not double insulated. The condition exposed employees to 120 volt electrical shock hazards.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor were employees provided with an alternative fall protection measure under another provision of paragraph 1926.501(b): (a)Extreme Construction LLC at 4460 Tennyson Street, Denver, CO 80212: On and before August 8th, 2015, the employer did not ensure that employees were using fall protection in the following instances: 1)Two employees were framing a deck on the third floor without the use of fall protection; and 2)Three employees were framing on the second floor and working near an open stairway hole without covering the hole, without installing guardrails, and without using personal fall arrest equipment. The condition exposed employees to fall hazards of up to 20 feet.
