29 CFR 1926.1053(b)(4): Ladders were used for purposes other than the purposes for which they were designed: On or about August 10, 2015, at the above addressed jobsite, while install Tyvek, an A frame ladder was being used for access to a working surface that was 140 inches above the ground below. The ladder did not reach the working surface and exposed the employees to a fall hazard when crossing from the ladder to the working surface.
