29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:Note:  As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system.  The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated    On or about August 5, 2015, at the front of the building, two employees in an aerial lift platform were not attached by lanyards to the platform of the aerial lift and were exposed to falls of greater than 6 feet to a lower level.

29 CFR 1926.502(e)(9): Positioning device systems were not inspected prior to each use for wear, damage, and other deterioration, and defective components were not removed from service:    On or about August 5, 2015, near side entrance of the building, a positioning device system had defective components and was not removed from service and was available for use in an aerial lift platform.  Two employees were exposed to falls of greater than 6 feet to a lower level when working in the aerial lift platform.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:   On or about August 5, 2015, at the front of the building, an employee was not protected from falling by guardrail systems with toe-boards, safety net systems, or personal fall arrest systems when working from the steep metal roof.  The employee was exposed to falls of greater than 6 feet to a lower level when working from the steep metal roof.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    On or about August 5, 2015, at the front of the building, the side rails of a ladder did not extend 3 feet above the metal roof.  An employee used the ladder to access the metal roof which exposes the employee to falls of greater than 6 feet to a lower level.     Javier Cerrito Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29 CFR 1926.1053(b)(1) which was contained in OSHA inspection Number 314935487, Citation Number 1, Item Number 2, and was affirmed as a final order on 2/3/2012 with respect to a workplace located at 14021 Korbyn Drive, Yukon, OK 73099.
