29 CFR 1910.219(i)(2): Bolt(s), nuts, or setscrews used in shaft coupling(s) extended beyond the flange of the coupling and were not covered with safety sleeve(s):  Hydro test truck:  On or about August 7, 2015, an employee used the truck mounted pump to perform a test on an oil well.  The pump's shaft coupling was not guarded and had projections.  This exposed the employee to the hazard of being caught by the rotating projections.
