29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: a) On or about 08/06/15, on the west side of the roof of a Multi unit Residential Building at 54 West Church St., Fairport, NY: Two employees engaged in roofing tearoff work on a steep roof were exposed to a fall of approx. 14 foot. Both employees were wearing full body harnesses, but neither was tied off to a lifeline or anchor point. NO ABATEMENT CERTIFICATION REQUIRED

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a) On or about 08/06/15, on the east side of the roof of a Multi-unit Residential Building at 54 West Church St., Fairport, NY: Four employees engaged in roofing work were exposed to a fall of 12 ft. 4 in. because the siderails of a red Louisville extension ladder used to access the roof from the ground, only extended 27 1/2 inches above the edge of the roof. NO ABATEMENT CERTIFICATION REQUIRED
