29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section: On August 10, 2015, an employee was exposed to an 18.2 foot fall hazard while working from a pump jack scaffold and the employer did not ensure that employees were protected from falling to a lower level. Abatement documentation is required of this item in accordance with 29 CFR 1903.19(d).

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. On August 10, 2015, an employee was exposed to a 11.9 foot fall hazard walking/working from the roof of a commercial building and the employer did not ensure employees were utilizing fall protection. Abatement documentation is required of this item in accordance with 29 CFR 1903.19(d).

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed. On August 10, 2015, employees were exposed to a fall hazard while accessing the roof from an extension ladder that extended approximately 6 inches past the roof and the employer did not ensure that the ladder extended at least three feet above the upper landing surface. Abatement documentation or certification is not required of this item.
