Employees working from a "Genie" aerial lift while performing remodeling activities on a bank building were not tied off or protected from a 26 foot fall hazard to the concrete below.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or warning line systems. Employees performing roofing type activities on a bank building roof were not wearing fall protection and exposed to an unprotected fall hazard of approximately 26 feet to the concrete below.
