29 CFR 1910.132(a): Protective equipment was not provided and used when necessary whenever hazards capable of causing injury and impairment were encountered and/or were not maintained in a sanitary condition:    a) Back Storage Rack Area and Fresh Chicken Refrigerated Areas:  Cleaning crews were not provided with and required to use appropriate foot protection (e.g., boots or equivalent) to clean floors with water and walking through chicken storage racks compartments surfaces where melting ice water along with grease and blood from raw poultry were present; on or about 8/4/15.    b) Truck Cleaning Area:   Cleaning crews were not provided with and required to use appropriate foot protection (e.g., boots or equivalent) to clean the floors of trucks when using water mixed with various cleaners including but not limited to Ben Ami Red Fox Cleaner (sodium metasilicate, sodium hydroxide), Cherry Kleen (2-butoxyethanol, sodium metasilicate); etc.; on or about 8/4/15.    c) Freezer Areas:  Cleaning crews and pickers working in the freezer were not provided with and required to use appropriate personal protective equipment (e.g., insulated jackets and hats or equivalent) against extreme-cold temperatures (approximately 5 to 10 deg. F); on or about 8/4/15.

29 CFR 1910.132(d)(1): The employer did not assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment:  a) Back Storage Rack Area and Refrigerated Fresh Chicken Storage Areas:  Cleaning crews cleaned inside racks and other areas where grease and blood from raw poultry were present.  The employer did not assess the workplace to determine if hazards were present which required the use of personal protective equipment (e.g., boots or equivalent over safety shoes); on or about 8/4/15.  b) Truck Cleaning Area:  Cleaning crew employees cleaned inside the back of delivery trucks using water and Ben Ami Red Fox Deodorizer (sodium hydroxide, sodium metasilicate) and Cherry Kleen (sodium metasilicate).  The employer did not assess the workplace to determine if hazards were present which required the use of personal protective equipment (e.g., boots or equivalent); on or about 7/30/15.  c) Fresh Chicken Refrigerated Storage Areas:  Warehouse workers picking and staging orders manually moved packages/boxes of beef, pork and boxes of fresh poultry packed in ice.  The employer did not assess the workplace to determine if hazards were present which required the use of appropriate hand protection (e.g., durable work gloves resistant to liquid penetration); on or about 8/4/15.

F); on or about 8/4/15.  c) Shipping and Receiving Areas:  Shipping and receiving employees loading and unloading trucks at the facility were not provided with and required to use durable work gloves; on or about 8/4/15.
