29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    (a) Packing room, Lines 1 and 2:  On or prior to 8/11/15, one or more methods of machine guarding was not provided to protect operators from exposure to pinch points and nip points created by the moving cassette(s) and associated components during operation of the PGX packing machines.    On 8/4/15,  an operator was injured while operating line 2 when her hand became caught in a pneumatic cassette push-bar mechanism as the bar returned, pinching her finger between the bar and the frame of the machine.

29 CFR 1904.32(a)(1):  The employer did not review the OSHA 300 log to verify the entries were complete and accurate, and correct any deficiencies identified:      (a) On or about 08/11/2015, OSHA Form 300 for calendar years 2014 to present:   Column D was not completed in sufficient detail in that it did not provide the specific date of the injury or illness.    For example, an injury recorded between 3/3/15 and 4/21/15 indicated as "packaging room", "hurt finger with machine"; the specific date of the injury was not indicated.      (b) On or about 8/11/2015, OSHA Form 300 for calendar years 2014 to present:   Column F was not completed in sufficient detail in that it did not provide a detailed description as to the specific nature of the injury or illness, parts of body affected, and object/substance that directly injured or made the person ill.    For example, an injury recorded on or about 8/4/15 indicated "hurt finger with machine", which does not specify the type of injury which occurred to the employee.    (c) On or about 03/03/2015, OSHA Form 300 for calendar years 2014 to present:   Column K was not completed in sufficient detail in that it did not provide the number of days away from work.    For example, an injury recorded on or about 4/21/15 indicated as "fractured forearm" and resulting in days away from work, the number of days away from work was not indicated.    (d) On or about 03/03/2015, OSHA Form 300 for calendar years 2014 to present:   Column L was not completed in sufficient detail in that it did not provide the number of days of job transfer or restriction.    For example, an injury recorded between 3/3/15 and 4/21/15, indicated as "hurt finger with machine" and resulting in job transfer or restriction, the number of days of job transfer or restriction was not indicated.
