29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:    a) Or about 8/07/15, on jobsite at 205 Carolina Street ,  Buffalo, NY:  Two employees had erected a tubular welded frame scaffold that was 7 sections high and 3 sections long. The employees were not trained by a competent person about the correct procedures for erecting, disassembling, and inspecting the scaffold, and how to recognize any hazards associated with this type of work.     b) On or about 8/10/15, on jobsite at 205 Carolina Street ,  Buffalo, NY:  Two employees exposed to a fall of approx.  33 ft. while grinding and repointing brickwork from platforms on a tubular welded frame scaffold were not trained about the pertinent OSHA scaffolding regulations,  nature of fall hazards, electrical hazards, load capacities, and proper material handling techniques associated with working from this type of scaffold. They also were not trained about the procedures to control or minimize those hazards.       ABATEMENT CERTIFICATION REQUIRED

29 CFR 1926.451(c)(1)(ii): Guys, ties, and/or braces were not installed  according to the scaffold manufacturer's recommendations or at the closest horizontal member to the 4:1 height and repeated vertically at locations of horizontal members every 20 feet (56.1 m) or less thereafter for scaffolds 3 feet ((0.91 m) wide or less, and every 26 feet (7.9 m) or less therafter for scaffolds greater than 3 feet (0.91m) wide.    a) Or about 8/07/15, on jobsite at 205 Carolina Street ,  Buffalo, NY:  Two employees had erected a tubular welded frame scaffold that was 7 sections high and 3 sections long. The height of the scaffold was approx. 33 ft., and the base width was 5 ft. , making the height to base width ratio 6.6. There were no guys, ties, or braces installed to restrain the scaffold from tipping.      ABATEMENT DOCUMENTATION REQUIRED

The gaps were approx. 10 to 12 inches wide.  ABATEMENT DOCUMENTATION REQUIRED
