29 CFR 1910.147(c)(4)(ii): Control of hazardous energy (lockout/tagout.) General. Energy control procedure. The procedures shall clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, the following: 29 CFR 1910.147(c)(4)(ii)(A): A specific statement of the intended use of the procedure; 29 CFR 1910.147(c)(4)(ii)(B): Specific procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy; 29 CFR 1910.147(c)(4)(ii)(C): Specific procedural steps for the placement, removal and transfer of lockout devices or tagout devices and the responsibility for them; and 29 CFR 1910.147(c)(4)(ii)(D): Specific requirements for testing a machine or equipment to determine and verify the effectiveness of lockout devices, tagout devices, and other energy control measures. On or about August 12, 2015, the employer failed to develop specific procedures for control of hazardous energy (lockout/tagout) for the authorized employees performing covered required duties on the Shredder machine. During routine activites, employees must open the protective door panels protecting the Shredders's point of operation to access and clear material jammed hangers and hanger components.

Locks, tags, chains, wedges, key blocks, adapter pins, self-locking fasteners, or other hardware shall be provided by the employer for isolating, securing or blocking of machines or equipment from energy sources. On or about August 12, 2015, the employer failed to provide locks or other protective materials, such as tags or lockout hasps, for employees performing covered duties by this part as authorized employees. Shredder machine operators open the protective doors and reach into point of operation area of the machine when clearing material jams without lockout/tagout protection from hazardous energy sources.

29 CFR 1910.147(d)(4)(i): Control of hazardous energy (lockout/tagout.) Lockout or tagout device application. Lockout or tagout devices shall be affixed to each energy isolating device by authorized employees. On or about August 12, 2015, during the covered activity involving clearing material jams from the Shredder machine, the employer did not assure that lockout/tagout control devices were applied to control hazardous energy. Isolating devices such as the disconnect box switch, did not have a tag and/or a lock, affixed with the isolating electrical disconnect box appropriately utilized or operated in the "off" position to control hazardous energy by isolating the machine. While clearing jams, the Shredder machine operator must reach into the panel doors to access the point of operation area to remove jammed hangers and hanger components without lockout protection.

Shredder machine employees perform covered activities under this part when clearing material jams from the point of operation area in the machine.

29 CFR 1910.37(a)(3): Exit routes must be free and unobstructed. No materials or equipment may be placed, either permanently or temporarily, within the exit route. The exit access must not go through a room that can be locked, such as a bathroom, to reach an exit or exit discharge, nor may it lead into a dead-end corridor. Stairs or a ramp must be provided where the exit route is not substantially level. On or about August 12, 2015, the employer had blocked the exit route and the exit door itself by boxes of hangers that were stacked and covered the entire pathway to the emergency exit door located on the east wall in the southeast corner of the facility. Sewing Collections Inc. was previously cited for a violation of this occupational safety and health standard which was contained in OSHA inspection number 842184, citation number 1 item number 2 and was affirmed as a final order on April 17, 2013, with respect to a workplace located at 2140 Refugee Road, Columbus Ohio.
