29 CFR 1926.20(b)(2): The employer did not initiate and maintain a safety program which provides for frequent and regular inspections of jobsites, materials, and equipment to be made by a competent person:   On or about August 12, 2015, during the installation of roofing material at 29517 Wolf Rd, Bay Village, Ohio 44140; the employer did not conduct frequent and regular inspections of the job site where employees were exposed to fall hazards and falling debris from working surfaces with a height of approximately 8.9 feet.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   On or about August 12, 2015, during the installation of roofing material at a job site located at 29517 wolf Rd, Bay Village, Ohio 44140; the employer did not ensure employees working on a roof of a single family home were protected from a fall of approximately 8.9 feet by utilizing a fall protection system.    The Hostetler Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13) Residential Construction, which was contained in OSHA inspection number 911459, citation number 1, item number 3 and was affirmed as a final order on 2/6/2014, with respect to a workplace located at 1691 Ledge Road, Hinckley, Ohio 44233.
