29 CFR 1910.132(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers were not provided and used where necessary by reason of hazards of processes or environment encountered in a manner capable of causing injury:    a) On or about August, 12 2015, personal protective equipment such as fall protection was not provided for employees exposed to a fall hazard of 18ft 6in into the elevator shaft  after manually opening an elevator door to check if the jams had been painted.
