29 CFR 1926.451(g)(4)(ii): The top edge height of toprails or equivalent members on supported scaffolds manufactured or placed into service after January 1, 2000 were not installed between 38 inches (0.97 m) and 45 inches (1.2 m) above the platform surface.  On or about August 18, 2015 at the south end of the work site where an employee was exposed to a fall hazard of more than 10 feet while working from a scissors lift without the toprail or equivalent being 38 to 45 inches above the platform surface.

29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous:    On or about August 18, 2015, at the south end of the work site where an employee was exposed to electrical hazards while using an electrical extension cord that was observed to have the grounding pin broken.
