29 CFR 1910.134(e)(1): The employer did not provide a medical evaluation to determine the employee's ability to use a respirator, before the employee was fit tested or required to use the respirator in the workplace.    This violation occurred on or about August 16, 2015 and at times prior thereto at DrillChem Drilling Solutions where the employer failed to ensure that medical evaluations were completed for employees that are required to use 3M 9211 N95 filtering facepiece respirators.      Pursuant to 29 C.F.R. 1903.19, within (10) calendar days of the abatement date the employer must submit documentation showing that it is in compliance with the standard, including the description of the steps that employees get a medical evaluations prior to use filtering facepiece.

This violation occurred on or about August 16, 2015 and at times prior thereto at DrillChem Drilling Solutions where the employer failed to ensure that employee(s) using tight-fitting or filtering facepiece respirators were fit-tested when required to use 3M 9211 N95 filtering facepiece respirators.      Pursuant to 29 C.F.R. 1903.19, within (10) calendar days of the abatement date the employer must submit documentation showing that it is in compliance with the standard, including the description of the steps that the employer will take to fit-test employees prior to use filtering facepiece.

29 CFR 1910.1200(g)(2): The chemical manufacturer or importer preparing the safety data sheet did not ensure that it is in English (although the employer may maintain copies in other languages as well), and includes at least the following section numbers and headings, and associated information under each heading, in the order listed (See Appendix D to � 1910.1200 Safety Data Sheets, for the specific content of each section of the safety data sheet)  The violation occurred on or about August 16, 2015 and at times prior thereto at DrillChem Drilling Solutions where the employer failed to ensure section numbers and headings and associated information under each heading of Safety Data Sheets listed in the order (1910.1200 (g)(2)(ii-x)) including but not limited to the following chemicals.  a)     Combat b)     DL-Glide c)     Drill-Beads d)     Drill-Lube GT e)     Drill-Lube TFH  Pursuant to 29 C.F.R. 1903.19, within (10) calendar days of the abatement date the employer must submit documentation showing that it is in compliance with the standard, including the description of the steps that section numbers and headings and associated information under each heading of Safety Data Sheet, listed in the order, in accordance with the Appendix D of 1910.1200.
