29 CFR 1910.147(c)(7)(i)(B): Affected employees were not instructed in the purpose and use of the energy control procedure:  a)  Production Area; On or about August 5, 2015 and at times prior thereto the employer exposed employees to potentially hazard energy sources in that employees were not provided training in lock out tag out as an affected employee which explained the purpose and use of energy control procedures on equipment.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:  a)  Facility wide; On or about August 5, 2015 and at times prior thereto the employer exposed employees to hazardous chemicals and substances that include but are not limited to welding gas, welding wire, oil, and grease without providing information and training on these chemicals and substances as required by the standard.

29 CFR 1910.1200(h)(3)(iv):   The details of the hazard communication program developed by the employer, did not include an explanation of the  safety data sheet, including the order of information and how employee could obtain and use the appropriate hazard information:  a)  Plant Wide; On or about August 5, 2015 and at times prior thereto the employer exposed employees to chemical hazards in that employees were not provided the new information and training on the UN Global Harmonized System (GHS) to include Safety Data Sheets (SDS).
