29 CFR 1926.501(b)(13):  Employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were other provisions of paragraph (b) of this section used to provide an alternative fall protection measure:    Jobsite located at 3904 370 Plaza, Bellevue, NE 68123: The employer is failing to ensure that employees engaged in residential construction activities are protected from falls while working at heights greater than 6 feet above lower levels. On or about August 15, 2015; employees of Jimenez Contracting, Inc. were observed performing residential asphalt shingle installation without conventional fall protection while exposed to fall distances of approximately 30 feet.

29 CFR 1926.503(b)(1):  The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer:    For the jobsite located at 3904 370 Plaza, Bellevue, NE:  The employer is failing to provide a written fall protection training certification that included the signatures of the person(s) who conducted the training or the signature of the employer; for each of the employees working on the roof.   On or about August 15, 2015, employees of Jimenez Contracting, Inc. were observed working at heights greater than 6 feet and not protected from fall hazards by acceptable measures.
