29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by an employee working from an aerial lift:  Residential building under construction; East Side, Snorkel Aerial Lift (Model: TB-42JD2, S/N: MR06084), On or about August 20, 2015, an employee was exposed to a fall approximately 13 feet in height to the paved ground level below, while cutting slots in a concrete brick wall of a commercial building from an aerial lift without wearing a harness.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  Residential building under construction; East Side, Snorkel Aerial Lift (Model: TB-42JD2, S/N: MR06084), on or about August 20, 2015, employees were exposed to a fall hazard of approximately 13 feet in height to the paved ground level below, when working in a aerial lift without wearing a harness while not being trained on the procedures to recognize fall hazards.
