29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above the lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, nor was an alternative fall protection plan which meets the requirements of 29 CFR 1926.502(k) developed and implemented:     a) 527 East Woodbridge Avenue, Avenel, NJ 07001: Employees were exposed to fall hazards when allowed to perform roofing operations at a height of approximately 10'10" from the roof eave to the ground below with no fall protection in place.    Condition observed on or about August 20, 2015.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:     a) 527 East Woodbridge Avenue, Avenel, NJ 07001: Employees were allowed to use a ladder that was not secured and did not extend 3 feet above the working surface to allow for safe access on and off the roof.     Condition observed on or about August 20, 2015.
