29 CFR 1926.501(b)(13):  Each employee engaged in residential construction activities 6 feet or more above lower levels were not protected by guardrail systems, safety net system, personal fall arrest system, or other approved alternative fall protection system:      (a)   Executive Coatings and Contracting Corp. at 7555 E Warren Dr, Denver, CO 80231:  On or before August 14, 2015 the controlling employer did not ensure that subcontractor employees were protected from injury, in that four roofers installing shingles on a three-story apartment complex, with a roof pitch of approximately 7-in-12, were not utilizing a fall protection system.  This condition exposed the subcontractor employees to a fall of approximately 30 feet.      (b)  Executive Coatings and Contracting Corp. at 7555 E Warren Dr, Denver, CO 80231:  On or before August 14, 2015 the controlling employer did not ensure that subcontractor employees were protected from injury, in that a roofer carrying a bundle of shingles on a three-story apartment complex, with a roof pitch of approximately 7-in-12, was not utilizing a fall protection system.  This condition exposed the subcontractor employee to a fall of approximately 30 feet.
