29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l):  Water jet shop:  On or about August 20, 2015, employees were exposed to a stuck-by hazard, while employees were allowed to operate forklifts prior to receiving training and prior to the employer certification of operators' competency.

South side of Cutting shop:  On or about August 20, 2015, employees were exposed to a caught-in hazard, while operating a Hyd-Mech bandsaw, Model DM6, and cutting metal parts, while the non-working portion of the blade of the bandsaw was not enclosed or guarded.

29 CFR 1910.253(b)(2)(ii): Assigned storage spaces for cylinders was not located away from elevators, stairs, or gangways where cylinders could not be knocked over or damaged by passing or falling objects, or subject to tampering by unauthorized persons:  a)  East inside exterior wall of Water jet shop:  On or about August 20, 2015, employees were exposed to a struck-by hazard, due to a compressed gas cylinder, N.O.S., UN 1956, which was not secured and subject to being knocked over.  b)  West inside exterior wall of Water jet shop:  On or about August 20, 2015, employees were exposed to a struck-by hazard, due to a compressed gas cylinder, N.O.S., UN 1956, which was not secured and subject to being knocked over.  c)  East inside exterior wall of Cutting shop:  On or about August 20, 2015, employees were exposed to a struck-by hazard, due to a compressed gas cylinder, Oxygen., UN 1072, which was not secured and subject to being knocked over.  d)  East inside exterior wall of Cutting shop:  On or about August 20, 2015, employees were exposed to a struck-by hazard, due to a compressed gas cylinder, Non-Flammable gas, which was not secured and subject to being knocked over.
