29 CFR 1926.1407(g): The employer failed to ensure that at least one electrocution hazard warning was conspicuously posted in the cab within view of the operator and at least two were posted on the outside of the equipment:   a)  At Pavo Road Well Site, on or about August 19, 2015, the employer did not ensure that a an electrocution warning decal, i.e., Danger Electrocution Hazard posted inside the cab and at least two on the outside of the 2001 Freightliner Truck Crane as a result exposing employees to an electrical shock hazard.

29 CFR 1926.1417(c)(1): The procedures applicable to the operation of the equipment, including rated capacities (load charts), recommended operating speeds, special hazard warnings, instructions, and operator's manual, were not readily available in the cab at all times for use by the operator:   a)  At Pavo Road Well Site, on or about August 19, 2015, the employer did not ensure that a load chart was readily available on the 2001 Freightliner Truck Crane as a result the operator could not determine load transactions.
