29 CFR 1910.178(l)(4)(iii): An evaluation of each powered industrial truck operator's performance was not being conducted at least once every three years.  a) Facility, 301 Ritztex Road, Montrose, PA: Employees operated powered industrial trucks (forklifts) daily and did not receive an evaluation of their ability to safely operate the equipment a minimum of at least once every three years. Employees were exposed to possible struck-by and crushed-by hazards from forklifts, as observed on or about August 25, 2015.  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation.

29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use.  a) Battery Charging Area, Shipping Department, 301 Ritztex Road, Montrose, PA - Employees recharged, added water, or added corrosive material (battery acid) to powered industrial vehicle (forklift) batteries in an area with an inoperable eyewash station. Employees were exposed to chemical exposure hazards to the eyes and face, on or about August 25, 2015.  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation.
