Belle Glade, FL 33430, employer did not provide a guard for a slicer machine Urschel, Model Diversacut Sprint serial number 383 food hopper, where employees were exposed to an amputation hazard while working at point of operation with a rotator impeller.

Belle Glade, FL 33430, the employer did not ensure lock out tag out procedure were followed, employees exposed while performing planned maintenance to a 480-volt electric panel located in the Pre-cooler area.

Belle Glade, FL 33430, employees were exposed to potential electrical shock and or burns from energized electrical wires while using a temporary extension cord running through the window and wall as a substitute for fixed wiring of the air conditioner which is located inside the Pre-cooler control room.
