Spaeder Company Inc.; 410 Seaward Avenue, Bradford, PA - On or about 8/25/15, an "A" frame ladder was closed, in a folded position and being used as an extension ladder.   Employees were using the 8 ft. ladder for access and egress of a concrete trench.
