29 CFR 1926.850(a): An engineering survey was not performed by a competent person to determine the conditions of the framing floors and walls and the possibility of unplanned collapse of any portion of the structure prior to permitting employees to start demolition operations. a) 151 Van Pelt Avenue, Staten Island, NY 10303- Employer failed to perform an engineering survey prior to beginning demolition work of an existing building, exposing employees to caught-in, fall and struck-by hazards. Condition occurred on or about August 28, 2015.

29 CFR 1926.854(d): Structural or load-supporting members on any floor were cut or removed prior to all stories above such a floor had been demolished and removed: a) 151 Van Pelt Avenue, Staten Island, NY 10303 - Employer allowed load-bearing structural support poles to be removed prior to the roof being removed or braced during demolition of an existing structure, exposing employees to crush, fall and struck-by hazards. Condition occurred on or about August 28, 2015.
