29 CFR 1926.451(g)(1)(iii): Each employee on a crawling board (chicken ladder) was not protected by a personal fall arrest system, a guardrail system (with minimum 200 pound toprail capacity), or by a three-fourth inch (1.9 cm) diameter grabline or equivalent handhold securely fastened beside each crawling board;  Location: site On or about 8/31/15, employees working on a steep roof more than 6 feet above a lower level using a crawling board were not provided fall protection.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  Location: roof  On or about 8/31/15, employees were not protected from falling while working on a 8-12 pitch roof more than 6 feet above a lower level without fall protection.
