29 CFR 1915.13(b)(9): Portable lamps or other electric devices which were not explosion-proof and approved by a National Recognized Testing Laboratory (NRTL) for hazardous locations were used in spaces not certified as "Safe for Workers"� during cleaning or other cold work in a confined or enclosed space:     On or about August 19, 2015, employees were exposed to the hazard of a flash fire while working in a flammable atmosphere with a cellphone used for light.
