29 CFR 1926.850(h): When debris was dropped through holes in the floor without the use of chutes, signs warning of the hazard of falling materials were not posted at each level and the required barricades were not provided:     a)  First Floor - Workers passing next to a floor penetration were exposed to falling concrete debris from an active demolition operation for the area immediately underneath the penetration was not provided with hazard warning signs on either floor and a Caution tape barricade was located only 4 ft. from the edge of where the concrete debris was dropped.
