29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by moving parts:     a) CNC machining area, on or about September 1, 2015, and times prior thereto, the Cosmec CNC routers were not guarded to prevent operators and other employees in the area from being caught in and/or struck by the moving gantry and router head.  Pinch points were created between the machines table and moving parts.

29 CFR 1910.334(a)(2)(ii): If there is a defect or evidence of damage that might expose an employee to injury, the defective or damaged item was not removed from service, and no employee may use it until repairs and tests necessary to render the equipment safe have been made:  a) Flaming Area, on or about September 1, 2015, and times prior thereto, extension cords, used supply power to the industrial fans, were damaged in that the outer sheathing was melted and cracked, exposing the inner wiring.   b) Cleaning, Press and CNC Areas, on or about September 1, 2015, and times prior thereto, the power cords of the pedestal fans were damaged in that the outer sheathing was cracked and torn, exposing the inner wiring.
