29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area: (Construction Reference: 1926.59)    a.) Windsong Medical at 55 Spindrift Drive - On or about 9/2-9/3/15,  five (5) employees working indoors for several hours cutting concrete, with a Husqvarna (FS400LV-LPG) Concrete Saw (propane-powered), were overcome by carbon monoxide resulting in one (1) employee being hospitalized overnight.  Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area.    ABATEMENT DOCUMENTATION REQUIRED
