29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):    On or about at 9/1/15 located at 6825 Wesley Hughes Rd, Cumming, GA: Employees were installing Handsplit Red Cedar roofing Shakes on the 10/12 pitch roof of a house that was under construction while working at the edge, the employer did not ensure that fall protection was used by the employees while exposed to fall hazards of approximately 40 feet.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On or about at 9/1/15 located at 6825 Wesley Hughes Rd, Cumming, GA: Employees were exposed to fall hazards, while performing roofing work, employees were using a portable extension ladder to gain access to the 10/12 pitch roof, and the ladder was only 24 inches above the landing surface.
