29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   On or about at 9/2/15 located at 615 Camden Park PL, Alpharetta, GA: A employees was installing roofing shingles on the 12/12 pitch roof of a house that was under construction while working at the edge, the employer did not ensure that fall protection was used by the employee while exposed to fall hazards of approximately 35 feet.

The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head.  On or about at 9/2/15 located at 615 Camden Park PL, Alpharetta, GA: Employees were installing roofing shingles on the 12/12 pitch roof of a house that was under construction while working at the edge, the employer did not ensure that the lanyards used for fall protection were attached to the back and centered at the shoulder. Employees were exposed to fall hazards of approximately 35 feet.
