29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  On or about September 4, 2015 at the jobsite located at or near 4120 NE Timber Lake Dr in Lees Summit, MO employees were exposed to a struck by hazard in that they were not wearing protective helmets while work was going on above them.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  On or about September 4, 2015 at the jobsite located at or near 4120 NE Timber Lake Dr in Lees Summit, MO employees were exposed to a fall hazard in that they were working on a steep roof without an approved form of fall protection.
