29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and moving machine parts:    Well site:  On or about September 3, 2015, and times prior to, an employee operated a pumping unit during the repair of an injection well.  The unit triplex pony rods movement was not adequately guarded exposing the employee to the hazard of being struck by projecting moving rod components.
