29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure:  a)  Jobsite, Harmony Dwellings 9th Ave & 9th Street, Beaver Falls, PA:  On or about September 2, 2015, an employee was re-facing a chimney on a residential roof and was not protected from falling.  The fall hazard to the employee was approximately 16-feet from peak to the ground.  The roof pitch was 4 in 12.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee who might be exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:   a)  Jobsite, Harmony Dwellings 9th Ave & 9th Street, Beaver Falls, PA: On or about September 2, 2015, an employee was re-facing a chimney on a residential roof without fall protection and had not been trained to recognize the dangers of falling and how to protect against them.
