One employee used the cross bracing to access a scaffold's work levels exposing the employee to falls.

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:  Tank farm area:  On or about September 3, 2015, employees engaged in steel erection activities worked from mobile scaffolds without fall protection.  This exposed them to falls exceeding 10 feet.

29 CFR 1926.760(a)(1): Each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems:  Tank farm area:  On or about September 3, 2015, employees engaged in steel erection worked from an existing roof with out using fall protection systems.  This exposed them to the hazard of falling over 15 feet to the ground level.
