Establishment: The employees, who were required to work on the second floor storage mezzanine of the warehouse were not protected from the hazard of falling over nine (9) feet to the floor by the use of standard railing system or an equivalent.

Establishment: The employer did not ensure through proving information and training and performing evaluation that the operator of the electric powered forklift truck (PIT) was competent to operate the PIT safely at all times.
