OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that employees were exposed to hazard of electrical shock or electrocution in that the effectiveness of the insulation was not tested:    a) For employees working from the bucket of the Altec bucket truck, model AA600L, serial number 0405-V1125, US DOT 981578, on or about September 9, 2015, the employer had not conducted a dielectric l test on the bucket truck at least annually. The last dielectric testing was done on June 28, 2014.

1926.960(c)(1)(iii): The employer did not ensure that no employee approached or took conductive objects closer to exposed energized parts than the employer's established minimum approach distance (MAD) or the distance computed by Table V-2:      On or about September 9, 2015 the employer did not ensure that employees exposed to 13Kv phase to phase line voltage maintained a MAD of 2.13 feet as required by Table V-2 or that employees were insulated from the energized part by rubber insulating gloves or rubber insulating gloves and sleeves worn in accordance with standard 1926.960(c)(2) which constitutes insulation of the employee from the energized part upon which the employee is working when the employee did not have control of the part in a manner sufficient to prevent exposure to un-insulated portions of the employee's body. Likewise, energized parts were not insulated from the employee and from any other conductive object at a different potential.    Or, in the alternative:    29 CFR 1910.269(l)(3)(iii): The employer did not ensure that no employee approached or took any conductive object closer to exposed energized parts than the employer's established minimum approach distance or the distance computed by Table R-3.    On or about September 9, 2015 the employer did not ensure that employees exposed to 13Kv phase to phase line voltage maintained a minimum approach distance (MAD) of 2.13 feet as required by Table R-3 or that employees were insulated from the energized part by rubber insulating gloves or rubber insulating gloves and sleeves worn in accordance with standard 1910.269(l)(4) which constitutes insulation of the employee from the energized part upon which the employee is working when the employee did not have control of the part in a manner sufficient to prevent exposure to un-insulated portions of the employee's body. Likewise, energized parts were not insulated from the employee and from any other conductive object at a different potential.
