29 CFR 1910.178(q)(1): Any power-operated industrial truck not in safe operating condition was not taken out of service, and/or repairs were not made by authorized personnel:    a)  On or about September 9, 2015, the employees were exposed to struck by hazards and/or crushing hazards in that the employer did not take the Hyster H60XM forklift out of service when the tires for the forktruck were in need of replacement.      Abatement Note:  Abatement certification and documentation are required for this item (see enclosed "Certification of Corrective Action Worksheet").
