29 CFR 1910.147(c)(4)(ii): The energy control procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D):  On or about September 3, 2015, at the Hydrodeoxygenator (HDO) Unit, the employer did not ensure that written lock out/tag out procedures contained specific steps and techniques for locking out Valve 5208 or the same type of valve (solenoid) including but not limited to identifying its dual energy sources (electricity and air), how to isolate the electrical power source and its location, how to isolate the air source and its location, how to dissipate any residual electrical and air energy, and how to verify all energy sources are isolated to prevent the release of highly hazardous chemicals (hydrogen gas).

29 CFR 1910.147(d)(6): Prior to starting work on machines or equipment that had been locked out or tagged out, the authorized employee did not verify that isolation and deenergization of the machine or equipment had been accomplished.    Before its employees commenced work on a piping system on or about September 3, 2015 at the REG Hydrodeoxygenator (HDO) Unit, the authorized employee of the contractor employer, Excel Contractors, took steps to verify that isolation and de-energization had occurred, which included the witnessing of verification steps performed by the facility owner.  But the verification steps taken were insufficient to verify that a solenoid operated valve (SOV) selected and utilized by the facility owner as an energy isolation point had completely isolated the piping system from hazardous energy (hydrogen gas.)"
