On or about 09/10/15, at the site of 75-58 196th Street. Fresh Meadows, NY     Employees working on unprotected ladder jack scaffolds were exposed to the hazard of falling approximately 14 ft. to the ground.     ABATEMENT VERIFICATION IS REQUIRED PURSUANT TO 29 CFR 1903.19
