29 CFR 1926.1053(b)(2): Employee(s) using ladders were carrying object or loads that could cause them to lose balance and fall: On or about September 09, 2015, and at times prior thereto, one worker using a 21 foot telescoping multi-ladder was climbing the ladder while carrying hardie board siding and a pneumatic hammer, exposing the employee to a fall hazard up to approximately 21 feet.

29 CFR 1926.1060(a): The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would train each employee in the procedures to be followed to minimize hazards related to ladders and stairways: On or about September 09, 2015, and at times prior thereto, one worker using a 21 foot telescoping multi-ladder was climbing the ladder while carrying hardie board siding and a pneumatic hammer and had not been trained on how to safely use the ladder, exposing the employee to a fall hazard of approximately 21 feet.
