An employee entered the de-barker machine area to clean tree bark from the internal segments of the machine. The employee was exposed to the de-barker head assembly, roller assembly and lift arms to the head-saw conveyor. 2. An employee entered the head-saw machine to clean out wood shavings from the platen area and double circular saw blade area. The employee was exposed to the double circular saw blades and the lumber feed assembly.

The operator platform to the de-barker machine did not have guarding at the north side near the de-branched tree conveyor and the west side entry into the de-barker head, roller assembly and lifting arms to the head-saw conveyor. 2. The west side of the inclined roll out conveyor to the Brewer horizontal re-saw had a section of guarding approximately 24 inches long that was missing over the roller chain.
