29 CFR 1926.100(a):     Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:      A) Sidewalk: - An employee was exposed to head injuries while working below a moving Komatsu Excavator. The employee was not protected by a hard hat or protective helmet, on or about August 4th, 2015.

29 CFR 1926.602(a)(9)(ii):     Earth moving or compacting equipment which had an obstructed view to the rear was operated in reverse gear; such equipment did not have in operation a reverse signal alarm distinguishable from the surrounding noise level or did an employee signalled that it is safe to do so.        A)  Exterior, Sidewalk: Employees were exposed to struck by injuries while working behind and near a Skid steel loader that did not have an operable/audible backup alarm, on or about August 4th, 2015.
