29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level:    (a)  FRONT OF HOUSE, ON TOP OF PORCH ROOF - Employee(s) installed siding on the front of the house, and they worked from a scaffold plank at a height of approximately sixteen feet, and the employee(s) were not protected by any means of fall protection, thereby exposing the employee(s) to possible injuries, on or about September 11, 2015.     Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR 1926.1053(b)(4): Ladder(s) were used for purposes other than the purpose for which they were designed:    (a)  FRONT OF HOUSE, ON TOP OF PORCH ROOF - Employee(s) installed siding on the front of the house, and they worked from a scaffold plank that had been erected between the roof of the house and the third rung of a Werner six (6) foot step ladder, thereby exposing the employee(s) to fall hazards of approximately sixteen(16) feet, on or about September 11, 2015.    Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
