Temporary cord was plugged into a 110 receptacle inside and passed straight downward through a hole in the floor, traveled under the floor and back up through another hole under the table.    b)  Instance #2: DPR Hardin Construction - WGI Joint Venture at 3280 Virginia Beach Blvd, Virginia Beach VA 23455 at NEXCOM Headquarters:  On or about 09-14-2015 and times prior thereto, was using a temporary extension cord to supply power to another smaller extension cord.  The two temporary extension cords were "daisy-chained" together.  Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employee(s) and their representatives were informed of this abatement.
