29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:    On or about September 14, 2015, the employer failed to provide adequate fall protection for employees. An employee working at the edge of a walking/working surface with an unprotected side did not have adequate fall protection. The employee was working more than 8 feet above the next level. The employee removed an A-frame ladder, which was acting as a mobile barrier guard, to access the edge of the walking/working surface.
